I am filling out the online on arrival visa form and there are two fields "Last Airport" and "Last Port of Call". I was wondering what needs to be filled in these fields. I am travelling on Qatar Airways from USA via Doha.

This is the correct form for visa on arrival: https://nepaliport.immigration.gov.np/on-arrival/io01
Did you proceed ahead with this? Qatar Air flights arrive in Kathmandu airport, transiting Doha. Message again if confused.
